Applicant was a woman in her late 50’s who worked as an ER
Registration Clerk for a medical center.
Her injuries were to her Neck and bilateral extremities from her repetitive duties doing desk work and data entry.
Surgeries were recommended by the doctors in her case, and repeatedly denied for one reason or the other by Utilization Review and IMR.
